The fee will be at least $25.00, and the exact amount will depend on the complexity of the document. Additionally, there is a charge associated with filling out and/or notarizing forms. We appreciate your understanding regarding these fees, as they are necessary to cover administrative costs.
At our office, we prioritize the confidentiality of your medical records. Without your written consent, no information from your chart will be released to anyone. Should you require the transfer of your records to another physician, we kindly ask that you submit a written request. Please note that this process may take up to thirty days, and requests are handled in the order they are received. You also have the option to personally pick up copies of any test results. Unfortunately, we are unable to mail, fax, or provide copies of your results to anyone other than yourself. Please be aware that transferring records to individuals such as patients, lawyers, or certain insurance companies will incur a fee, which depends on the volume of the record. It is important to allow up to thirty days for records to be copied and mailed.
Regarding prescription refills after hours, we do not refill routine prescriptions during these times or on weekends. We kindly request that you regularly check your medications and ensure that you have an adequate supply. This policy applies to medications such as those for diabetes, hypertension, arthritis, and similar conditions. In case of an after-hours emergency, please contact the telephone number provided, and the voicemail will provide further instructions. Please allow 24 hours for prescription refill requests to be processed. It is important to note that our office closes at noon on Fridays, and any requests received after 11:00 am may not be processed until the next business day.
If you require a referral to a specialist, it is necessary for you to first come in and see the doctor. If you are seeing a specialist on a regular basis and they send us consult letters requesting your return visit, we may be able to facilitate this once the doctor has reviewed the notes and approves the referral. We kindly ask that you refrain from making an appointment to see a specialist until our staff has obtained the referral for you. Our office staff handles referrals to specialists, and it may take 48-72 hours for these referrals to be processed. After processing by our office, it sometimes takes an additional 48-72 hours to receive back an approval referral. Please be aware that referrals are processed in the order they are received, and we appreciate your patience in this matter.
